Solution:

If basic troubleshooting steps have already been taken and the problem perists, the issue may be caused by additional code added to the notes.ini file by antivirus software. 

  1. In the notes.ini file, remove any lines that start with:
  2. AddInMenus
  3. EXTMGR_ADDINS
  4. Rerun the validation test.
